Referring a patient with persistent pain
If you have a patient who has been living with chronic pain for months or years, has tried the standard routes without lasting relief, and whose scans or investigations do not fully explain their symptoms, they may be a good candidate for the No More Pain Programme.
Coenie de Groot is a registered Osteopath and certified pain reprocessing therapy practitioner. He works with patients experiencing a wide range of persistent pain conditions and neuroplastic symptoms, primarily through a 60-day online programme delivered worldwide
He is happy to receive referrals from GPs, Physiotherapists, Osteopaths, Occupational Therapist, other healthcare professionals and case managers. Either you or your patient can get in touch directly.
Which patients are suitable
The No More Pain Programme is designed for patients with persistent pain where a structural cause has either been ruled out or does not fully account for the level of symptoms. Suitable patients typically present with one or more of the following:
-
Chronic back pain, neck pain or widespread musculoskeletal pain persisting beyond three months
-
Fibromyalgia or central sensitisation syndrome
-
Chronic fatigue, migraines, tinnitus or other stress-related neuroplastic symptoms
-
Pain that varies significantly with mood, stress or life circumstances
-
Normal or near-normal imaging findings alongside significant functional impairment
-
A history of multiple treatments without lasting benefit
-
Long COVID symptoms including persistent pain, fatigue or brain fog
The programme is not suitable for patients with active inflammatory conditions, undiagnosed or unstable medical presentations, or pain with a clear structural cause that has not yet been appropriately investigated or treated.

About Coenie
Coenie de Groot holds a Master's degree in Osteopathy and has over 25 years of clinical experience in movement, posture and pain. He is a registered Osteopath and a SIRPA (Stress Illness Recovery Practitioners Association) certified practitioner, trained in pain reprocessing therapy and the mind-body approach to chronic pain recovery.
His work is informed by the research of Dr John Sarno, Dr Howard Schubiner and Dr David Hanscom, whose DOC (Direct Your Own Care) Method provides a structured approach to neural pathway retraining. His approach combines clinical osteopathic assessment with pain neuroscience education and pain reprocessing therapy.
As a registered Osteopath, Coenie brings clinical anatomy expertise that most PRT practitioners do not have. He is able to conduct an initial clinical assessment, understand the full musculoskeletal history, and ensure the programme is appropriate for each individual before beginning.
What the programme involves
The No More Pain Programme is a 60-day structured programme delivered primarily online. It combines pain neuroscience education with pain reprocessing therapy to help patients understand and address the neuroplastic mechanisms driving their pain.
It covers:
-
Pain neuroscience education tailored to the individual patient
-
Identification of the psychological, emotional and stress-related contributors to their symptoms
-
Practical techniques to break the fear-pain cycle and reduce central sensitisation
-
Neural pathway retraining through somatic tracking and related approaches
-
Tools to maintain recovery and prevent relapse long term
Most patients start to notice a meaningful reduction in symptoms within four weeks. The programme is tailored to each individual and can be adjusted to account for comorbidities or specific clinical considerations.
How to refer
There is no formal referral process. Either you or your patient can get in touch directly to find out whether the programme is appropriate and to ask any questions.
Coenie is happy to speak with referring professionals before a patient makes contact, if that would be helpful.
